Fees are due at each appointment, and are payable with cash, check or credit card. Please call regarding hourly rates.
I reserve a portion of my practice for those in need of a reduced fee due to financial hardship. If you qualify, we can discuss this at our first session.
I do not bill insurance companies or third parties. Upon request, I will provide you with an invoice to submit to your insurance company. I can not guarantee that you will be reimbursed for your time with me.
Kindly provide at least 24 hours notice if you are unable to keep your scheduled appointment. Doing so will help you avoid being charged for a missed appointment. In the event that an appointment needs to be rescheduled, my voice mail and email are available 24 hours a day.
